Privacy Policy

Effective date: October 26, 2026

ClearBridge Wellness PLLC (“ClearBridge Wellness,” “we,” “us” or “our”) respects your privacy. This Privacy Policy explains what information we collect when you visit clearbridgewellness.info (the “Website”), how we use it, and the choices you have.

Your health information is protected by HIPAA

When you become our patient, your health information is protected health information under the Health Insurance Portability and Accountability Act (HIPAA). How we use and share that information is explained in our Notice of Privacy Practices, not in this Privacy Policy. This Privacy Policy covers only your use of the Website.

Information we collect

  • Information you choose to send us. If you call or email us, we receive your name, your contact information and whatever you include in your message. Please do not include health information in email. Use the secure patient portal, or call our office for prescription refills.
  • Technical information. Like most websites, our hosting provider automatically records basic technical information when you visit, such as your IP address, browser type, the pages you view and the date and time of your visit. This helps keep the Website working and secure.
  • No health information through the Website. The Website has no forms that ask about your health, and it does not create user accounts.

Appointments, intake and the patient portal

Appointment requests, intake forms, secure messaging, billing and telehealth visits are handled through SimplePractice, a secure, HIPAA compliant practice management platform. When you select Request an Appointment or Patient Portal, you leave this Website. Information you provide there is protected health information. It is handled under our Notice of Privacy Practices and SimplePractice’s own privacy policy, and SimplePractice is required to protect it under a business associate agreement with us.

How we use information

  • To respond to your questions and requests
  • To schedule and coordinate your care
  • To operate, maintain and protect the Website
  • To meet our legal and professional obligations

We do not sell your information

We do not sell your personal information. We do not share it for targeted advertising, and the Website does not use advertising cookies, tracking pixels or session recording tools.

Cookies and outside services

The Website does not use advertising or analytics cookies. To display its typefaces, the Website loads fonts from Google Fonts, which receives your IP address in order to deliver them. If we add other services in the future, we will update this Privacy Policy first.

When we may disclose information

  • To service providers that help us run the practice and the Website, such as our hosting, email, phone and practice management providers. They may use the information only to provide services to us.
  • When required by law, such as in response to a court order or valid legal request.
  • When necessary to protect the health or safety of you or others, including in an emergency.

How we protect information

We use reasonable administrative, technical and physical safeguards to protect information, and the Website uses an encrypted (HTTPS) connection. No method of sending information over the internet is completely secure. Regular email is not a secure way to send health information, which is why we ask you to use the patient portal.

Children’s privacy

The Website is meant for adults, including parents and guardians. We do not knowingly collect personal information through the Website from children under 13. For patients under 18, a parent or legal guardian arranges care and completes intake through the patient portal. If you believe a child has sent us personal information through the Website, contact us and we will delete it.

Links to other websites

The Website links to other sites, such as SimplePractice, the 988 Suicide & Crisis Lifeline and government resources. Their own privacy policies apply when you visit them.

Your choices and rights

You may ask us to access, correct or delete personal information you have sent us through the Website, such as an email. Depending on the state where you live, you may have additional rights under state privacy laws, including Washington’s My Health My Data Act. Requests about your medical record are handled under HIPAA, as explained in our Notice of Privacy Practices. We may need to confirm your identity before completing a request, and we will never treat you differently for making one.

Changes to this Privacy Policy

We may update this Privacy Policy from time to time. When we do, we will post the new version here and change the effective date at the top.

Notice of Privacy Practices

THIS NOTICE DESCRIBES HOW MEDICAL INFORMATION ABOUT YOU MAY BE USED AND DISCLOSED AND HOW YOU CAN GET ACCESS TO THIS INFORMATION. PLEASE REVIEW IT CAREFULLY.

Effective date: October 26, 2026

ClearBridge Wellness PLLC is required by law to protect the privacy of your health information, to give you this notice of our legal duties and privacy practices, and to follow the terms of the notice currently in effect. When the law of the state where you receive care gives you more protection than federal law, we follow the more protective law.

Your rights

When it comes to your health information, you have the right to:

  • Get a copy of your health and billing records. Ask us for a paper or electronic copy. We will usually provide it within 30 days and may charge a reasonable, cost based fee.
  • Ask us to correct your records. If you believe information is incorrect or incomplete, ask us in writing. We may say no, but we will tell you why in writing within 60 days.
  • Request confidential communications. Ask us to contact you in a specific way, such as a certain phone number or address. We will say yes to all reasonable requests.
  • Ask us to limit what we use or share. We are not required to agree, but will consider your request. If you pay for a service in full yourself, you can ask us not to share information about it with your health insurer, and we will say yes unless the law requires us to share it.
  • Get a list of those we have shared your information with. You can ask for a list of the times we shared your information in the six years before your request, with whom and why. It will not include sharing for treatment, payment or health care operations, or sharing you asked us to make. We provide one list a year for free and may charge a reasonable fee for additional lists within 12 months.
  • Get a copy of this notice. You can ask for a paper copy at any time, even if you agreed to receive it electronically.
  • Choose someone to act for you. If someone is your health care power of attorney or legal guardian, that person can exercise your rights. We will confirm their authority first. For patients under 18, a parent or guardian usually acts for the child, but state law may give minors the right to consent to some care and to keep related information private.
  • File a complaint if you feel your rights were violated. Contact us using the information below, or file a complaint with the U.S. Department of Health and Human Services Office for Civil Rights at 200 Independence Avenue SW, Washington, D.C. 20201, by calling (877) 696-6775, or at www.hhs.gov/ocr/privacy/hipaa/complaints. We will never retaliate against you for filing a complaint.

Your choices

You can tell us your choices about what we share in these situations:

  • Sharing information with your family, close friends or others involved in your care or payment for your care
  • Sharing information in a disaster relief situation

If you are not able to tell us your preference, for example if you are unconscious, we may share your information if we believe it is in your best interest. We may also share information when needed to lessen a serious and imminent threat to health or safety.

We never share your information for marketing purposes or sell your information unless you give us written permission.

Psychotherapy notes

Psychotherapy notes are notes your provider may keep separately from your medical record about the contents of therapy sessions. In most cases, we need your written permission before sharing them.

How we typically use and share your information

  • To treat you. We use your information and may share it with other professionals who are treating you. For example, we may coordinate with your primary care provider or a pharmacy that fills your prescription.
  • To run our practice. We use your information to operate the practice, improve your care and contact you when necessary, such as appointment reminders.
  • To bill for your services. We use and share your information to bill and get payment from health plans or other entities.
  • With our business associates. Companies that help us run the practice, such as our electronic health record and telehealth provider, may receive your information. They are required by contract to protect it.

Other ways we may use or share your information

We are allowed or required to share your information in other ways, usually in ways that contribute to the public good. We must meet many conditions in the law before we can share your information for these purposes:

  • Public health and safety issues, such as reporting suspected abuse, neglect or domestic violence, and preventing or reducing a serious threat to anyone’s health or safety
  • Complying with the law, including when state or federal law requires it or the Department of Health and Human Services asks to confirm we are complying with federal privacy law
  • Health oversight activities, such as audits, investigations and licensing
  • Lawsuits and legal actions, in response to a court or administrative order or a valid subpoena
  • Law enforcement purposes, and special government functions such as military, national security and presidential protective services
  • Workers’ compensation claims
  • Coroners, medical examiners and funeral directors, when an individual dies
  • Health research, under strict privacy protections

We will not use or share your information in any way not described in this notice unless you give us written permission. If you give permission, you may change your mind at any time by telling us in writing.

Our responsibilities

  • We are required by law to maintain the privacy and security of your protected health information.
  • We will let you know promptly if a breach occurs that may have compromised the privacy or security of your information.
  • We must follow the duties and privacy practices described in this notice and give you a copy of it.

Changes to this notice

We can change the terms of this notice, and the changes will apply to all information we have about you. The new notice will be available on our website, in the patient portal and on request.

Consumer Health Data Privacy Notice

Effective date: October 26, 2026

Some state laws, including Washington’s My Health My Data Act, protect “consumer health data.” This is personal information that identifies a person’s past, present or future physical or mental health, including information showing that someone is seeking health care. This notice explains how ClearBridge Wellness PLLC handles consumer health data that is not covered by HIPAA. Health information we collect while caring for you as a patient is protected health information under HIPAA and is covered by our Notice of Privacy Practices instead.

Consumer health data we may collect

  • Information you include when you call or email us before becoming a patient, which may show that you are seeking mental health care
  • Basic technical information recorded when you visit our website, such as your IP address and the pages you view, which could indicate interest in mental health services

How we collect and use it

We collect this information directly from you, and automatically through our website hosting provider. We use it only to respond to you, schedule and coordinate care you have requested, and keep our website working and secure.

How we share it

We share consumer health data only with service providers that help us operate, such as our website host, email and phone providers and our practice management platform, and only so they can provide services to us. We may also disclose it when required by law. We do not sell consumer health data, and we do not use it for targeted advertising.

Your rights

Depending on where you live, you may have the right to:

  • Confirm whether we collect, share or sell your consumer health data, and access it
  • Receive a list of the third parties and affiliates with whom we have shared it
  • Withdraw your consent to our collection and sharing of it
  • Ask us to delete it
  • Be free from discrimination for exercising these rights

To make a request, contact us at (281) 638-5187 or hello@clearbridgewellness.info. We may need to confirm your identity first. We will respond within the time the law requires. If we deny your request, you may appeal by contacting us with “Appeal” in the subject line. If we deny your appeal, you may contact the attorney general of your state.
